Privacy Notice
Effective 7 October 2026. This notice describes the FORM early-access website and the current fitness app.
Early-access requests
When you submit the form, we store your email address, selected platform, request date and consent version to manage early access and send related updates. The website is hosted through Sites on Cloudflare infrastructure. Hosting providers may process technical logs to deliver and secure the service. This site does not add advertising analytics or tracking cookies.
Your app data
The core tracker stores fitness history and photos on your device. You control backups and exports. Maps and other optional services may receive the data required for the feature you use. Availability depends on the installed build and permissions.
Optional Claude coach
AI context sharing starts off. When you request assistance, selected context and text you type are sent directly to Anthropic. Your own API key is stored in the iPhone Keychain for native builds, outside fitness backups. Web previews hold it in session memory. Anthropic’s policies apply to requests sent to its API. Turning sharing off cannot retract requests already sent.
Your choices
You can decline early-access emails by not submitting the form. Once invitations begin, you can reply to the invitation sender to withdraw consent or request deletion of your early-access record. Local app history can be managed on your device. Do not submit sensitive health information through the website form.
Release transparency
FORM is an early-stage product. This notice will be updated as distribution, support contacts and service providers change. A verified company contact and legal entity information must be added before general commercial release.
